Sign up your child(ren) for all sessions, and receive a $50.00 credit per child on your session #7 bill.

To Register:

• Complete Registration Form
• Select Sessions and Time Slots
• Indicate T-Shirt Size
• Submit $150.00 deposit per child & $12.00 T-shirt fee.  (The deposit will be applied to your last scheduled session, providing all sessions signed up for were paid in full.)
• Once you are confirmed for your session selection, you are obligated to pay the full session fee. Failure to pay will result in forfeiture of your deposit.
• Submit your child's Health Form
  
  The State of Connecticut requires that the health form be in compliance wit the Shelton School system's requirements. Therefore, a photocopy of your child's health form from his school file is acceptable.
  
  If your child is entering kindergarten in September, we need a health form with the following information: Child's name, date of birth, up to date list of immunizations, a physical (must be within one year), doctor' signature and stamp.
  
  The completed and correct form must be on file before your child can attend summer camp.
